Beginning May 20, 2013, the Joint Health Management Board (JHMB) will be conducting a Dependent Verification Review for all benefit-eligible employees (both full-time and part-time) of the Fresno Unified School District. The JHMB will partner with the District’s Section 125 administrator, American Fidelity Assurance (AFA), to conduct the review. The review will take place through September 23, 2013.

JHMB and the District conducted a successful Dependent Verification Review in 2007. Due to current Health Care Reform legislation, experts believe that a Dependent Verification Review should be conducted often to maintain the integrity and compliance of the Health Care Plan.

From May 20 to September 23, 2013, you are required to meet with an AFA Benefits Reviewer to complete your Dependent Verification Review. This review is required even if you do not have dependents enrolled under your benefits coverage. We are seeking a true and complete review of all employees.

During the confidential one-on-one meeting with an AFA benefit reviewer, you will be asked to provide the documents listed under the eligibility guidelines (see page 3). Please be prepared to bring original or certified documents with you to your meeting with an AFA benefit reviewer. The documents you provide will be checked only by the AFA benefit reviewer, recorded on the audit form, and returned to you. Please do not send or take your documents to the District Office or any other location.

These documents are essential to confirm eligibility for health coverage. You can be assured that the District will not receive or retain copies of these documents. The District will only receive a copy of the dependent verification form from AFA. [/togg]

[togg title=”What Happens if You Do Not Meet with AFA?”]
Failure to meet with an AFA benefit reviewer and complete your eligibility review between May 20, 2013 and September 23, 2013 will affect your coverage. If you do not comply, your dependents will be terminated from the plan effective November 30, 2013. [/togg]

[togg title=”How to Schedule Your Session with AFA”]
AFA benefit reviewers will be available while school is in session from May 20 – June 13 and at summer school sites, the Education Center, CSEA, SEIU and FTA Offices from June 17 – August 13. Please review the Site Schedule for details about meeting dates at specific district locations. Please call AFA to schedule an appointment to complete the review at the date, time and location of your choice.

Additional enrollment dates will be scheduled during the Fall between August 14 and September 23. However, do not delay in scheduling your appointment, as these appointment dates and times will be going fast.

Your individual session with the AFA benefit reviewer will not take more than 20 to 30 minutes and will be much less if you do not have dependent coverage. During these confidential one-on-one sessions with the AFA benefit reviewer you will verify your information and the reviewer will be available to discuss benefits, Section 125 options, and other voluntary insurance programs that may be important to you.[/togg]

Once you have completed the dependent verification review portion of your meeting with AFA and have received receipt of your signed dependent verification form, you are under no obligation to continue the interview. You may be asked whether you would like to receive information from AFA regarding their voluntary services. It is your personal decision whether to remain for that portion of the discussion. The District has reached an agreement with AFA that they will not aggressively solicit our employees during these sessions. There is absolutely no obligation to purchase any American Fidelity products.[/togg]

This is Not an Open Enrollment Opportunity
This is only an eligibility verification review. You will have an opportunity to make changes to your plan at the next open enrollment beginning October 2013.
