Find A Provider

Navigating the healthcare market can be tough, but we believe finding the right provider for you and your family shouldn’t be. That’s why the Joint Health Management Board (JHMB) has partnered with key vendors to create quick guides, helping you find network providers for your medical, dental, vision, behavioral health, and acupuncture and chiropractic care needs. When reaching out to a provider, just have your insurance card ready, as it contains vital plan details like the group number and network, so your provider can efficiently answer your questions.

Benefits of Using
In-Network Providers

You can reduce your out-of-pocket costs and avoid unnecessary fees by using in‑network providers. You often pay much less for in‑network care because:

  • In-network providers agree to discount or reduce their fees for you as a member of the network.
  • Most non-hospital benefits are subject to only a copay. Whereas using an out-of-network provider means you pay a coinsurance of the “Usual,” “Reasonable,” and “Customary” cost for most benefits (generally more expensive than a copay).
  • You don’t have to file a claim—the provider does this for you.
  • A provider won’t bill for more than the fee agreed upon by the network. Out-of-network providers often “balance bill,” meaning they bill patients for the balance between what they want to charge for services and what the insurance company has already reimbursed them.

Medical Providers

Aetna choice POS II Network

Aetna’s dedicated microsite, www.AetnaResource.com/p/FresnoUSD, is your online resource for finding doctors, hospitals and other healthcare professionals that participate in the Aetna Choice POS II Network. For directions on how to use the microsite, refer to this guide.

Note: Make sure you are using the microsite when searching fro medical providers. Using other sources, including Aetna.com, will provide inaccurate search results based on the plan structures.

Kaiser Permanente

If you are enrolled in the Kaiser Permanente HMO medical plan, “My Doctor Online” makes it easy for you to choose the right doctors for you and your family! Follow these directions to search for the doctor who is right for you:

  1. Go to kp.org/mydoctor.
  2. In the top right section of the page, click on “Choose a Doctor.”
  3. In the “Search Doctors” section of the web page, click on “Go to Search.”
  4. In the “Search” section, under “Search for a Doctor In,” click the button next to the specialty you are looking for (e.g. adult medicine, obstetrics, and gynecology or pediatrics).
  5. You can then search available doctors (by distance or location).
  6. Click “Search.”

Dental Providers​

Delta Dental

If you are enrolled in the Delta Dental PPO plan, you can locate an in-network provider by visiting www.deltadentalins.com. Follow these directions to find a dental provider in this network:

  1. In the “Find a Dentist” box on the right side for the website’s home page, select your plan network: Delta Dental PPO.
  2. You can also specify:
    • The dentist’s city and/or zip code
    • The dentist’s name
    • The dentist’s specialty
  3. Click “Search.”

Delta Dental’s online directory also provides a map and driving directions to the dental office by clicking on the office address.

UnitedHealthcare Direct Dental Compensation

If you are enrolled in the UnitedHealthcare Dental plan, here is how you can locate a network provider:

  1. Visit www.myuhcdental.com. If you receive a pop-up window to register or login, ignore the message and exit the pop-up window by clicking outside of it.
  2. On the right side fo the page, click on “Find a Dentist.”
  3. Select “CA Select Managed Care Direct Compensation.”
  4. You can then search by address, city and state, zip code, or by provider or dental practice name.

Vision Providers

MESVision

To locate an MESVision network provider:

  1. Go to www.MESVision.com.
  2. Enter your user name and password, and click “Login.”
  3. Click on your Group (Company) name.
  4. Click on “Find a Doctor” tab at the top of the web page.
  5. Enter your zip code and click “Search” to gain access to a list of MESVision providers within a specified radius.
Halcyon Behavioral Health

Halcyon Behavioral Health’s provider panel is a local network of pre-screened, highly professional clinicians who provide the highest quality Mental Health services to employees and their dependents.

To find an Halcyon Behavioral Health provider, you have two options:

  • Option 1: Call
    • Call a Mental Health clinician at (888) 425-4800.
  • Option 2: Search online at www.fusdmhsa.com
    • Click on the “Provider Network” link.
    • Locate a provider using the search functions.

Behavioral Health Providers

Claremont EAP

FUSD employees and eligible dependents can access EAP Services by calling (800) 834-3773. Your Claremont Intake Counselor will do a brief assessment to help identify a provider that will fit your needs based on the presenting issue, your preferred locations/times, and any other personal preferences (i.e male/female, clinical orientation, any specific language needs, etc.).

You will be referred to a provider who is also part of the Avante network in case you need further treatment covered under the Behavioral Health Benefits that Avante provides. You can also begin the process online by going to www.claremonteap.com/help-request-form and completing the Help Request Form.

Acupuncture & Chiropractic Care Providers

PhysMetrics (formerly Chirometrics)

Acupuncture
No referral is required, and you may change providers at any time! To find a PhysMetrics network provider for Acupuncture services, you can:

  • Visit fusdacupuncture.com to find and view a list of contracted acupuncturists.
  • Call PhysMetrics Customer Services at 877-519-8839 from 8 a.m. to 5 p.m., Pacific Time, Monday through Friday.

Chiropractic Care
No referral is required, and you may change providers at any time! To find a PhysMetrics network provider for Chiropractic services, you can:

  • Visit fusdchiro.com to find and view a list of contracted acupuncturists.
  • Call PhysMetrics Customer Services at 877-519-8839 from 8 a.m. to 5 p.m., Pacific Time, Monday through Friday.

Become a WellPATH Champion

I understand the outlined roles and responsibilities of the Wellness Champion and will assist in the implementation and coordination of wellness initiatives. I agree to not only support the WellPATH Employee Wellness program by my participation, but to collaborate with my local management team to support the initiatives as well. I understand that the success of the program hinges on all employees being empowered to make informed decisions that positively impact their health. The WellPATH program can count on me to be a messenger and motivator and to help gather information from my co-workers, principals and managers to help foster a culture of wellness within Fresno Unified School District that supports good health.
