Wellness Event Request

The WellPATH Program will be primarily a request-based program that relies on employees’ assistance to coordinate events.  Requests should be submitted 4-6 weeks prior to the desired event date.

What to do

  • Secure approval from principal or site supervisor.
  • Secure a minimum of 15 interested participants.
  • Reserve an appropriate room.
  • Complete the Wellness Event Request Form and return it to the WellPATH program.

Who is Eligible?

Wellness seminars are free to benefit-eligible employees covered by the FUSD health plan.

Minimum Requirements

Unless otherwise indicated, class minimum is 15.  If the interest from employees at your worksite is too small to meet the minimum, consider partnering with other nearby schools or district worksites.  You can also contact the WellPATH office to try to locate other interested employees in your area.

Promotion and Registration

WellPATH will provide you with an electronic flyer to promote your event at your worksite and will facilitate the registration process.

Cancellation

Seminars with insufficient enrollment will be canceled at least 10 business days before the workshop is scheduled to occur.  In the event the workshop is cancelled, participants will be notified by the WellPATH office.
