Explore all of your potential plan benefits below to find information about the employee benefits and wellness programs described in the Benefits Info Guide and Plan Booklet. Your specific benefits are determined by the plan(s) chosen during enrollment, which can be found in your personal plan documents and insurance booklets.

PPO Medical Benefits

With a Preferred Provider Organization (PPO) plan, you have greater flexibility and choice to use both in-network and out-of-network providers. However, you are encouraged to receive services from the Aetna network of doctors, specialists, and facilities. By doing so, you obtain a higher level of benefit than if services were rendered from an out-of-network provider. Also, claim forms are submitted to the plan on your behalf when services are received from within the network.

HMO Medical Benefits

As a member of the Kaiser Permanente Health Maintenance Organization (HMO) under Kaiser HMO Plan C or the Kaiser Permanente Senior Advantage plan, you will receive your medical care from an integrated network of physicians and specialists at a medical office, medical center, or affiliated hospital near you.

Prescription Benefits

Prescription drug coverage for PPO Plans A and B are administered through Elixir for employees, non-Medicare retirees, and non-Medicare dependents. For Medicare-eligible retirees and/or dependents, prescription drug coverage is administered through Aetna. If you are enrolled in HMO Medical Plan C or the Medicare Advantage HMO Plan, your prescription coverage is provided by Kaiser Permanente.

Dental Benefits

You and your eligible dependents have the opportunity to enroll in a Dental Health Maintenance Organization (HMO) plan offered by UnitedHealthcare or a Dental Preferred Provider Organization (PPO) plan offered by Delta Dental. Learn more about how to use your benefits below, according to your plan selection.

Vision Benefits

Vision coverage for members enrolled in Medical Plan A or B is offered by VSP Vision as a Preferred Provider Organization (PPO) plan. If you are enrolled in Medical Plan C, your vision coverage is offered by Kaiser Permanente.

Supplemental Benefits

Supplemental benefits, including the Wellness Program, Employee Assistance Program (EAP), FSAs, and life insurance plans, are available to full-time employees, retirees, and dependents. Some benefits are included with plan benefits, while others may be added at your own expense during enrollment.

Become a WellPATH Champion

I agree to represent my school/department as the Wellness Champion for this school year. I understand the outlined roles and responsibilities of the Wellness Champion and will assist in the implementation and coordination of wellness initiatives. I agree to not only support the WellPATH Employee Wellness program by my participation, but to collaborate with my local management team to support the initiatives as well. I understand that the success of the program hinges on all employees being empowered to make informed decisions that positively impact their health. The WellPATH program can count on me to be a messenger and motivator and to help gather information from my co-workers, principals and managers to help foster a culture of wellness within Fresno Unified School District that supports good health.

I also understand that I may be asked to assist in scheduling rooms and programs (including wellness screenings, seasonal influenza program) at my location, displaying WellPATH brochures, posters, etc. in appropriate areas, and encouraging my co-workers to be involved in the WellPATH Employee Wellness program.
