Using Your Card

Your insurance card is more than just a piece of paper or plastic—it’s your key to accessing the healthcare benefits you’re entitled to as a District employee, retiree, or dependent. This page will guide you through the essentials of using your insurance card effectively, whether you have a physical cardstock or laminated ID card, or a printable digital version.

Note: the location of the information on your ID card varies depending on your health care plan.

Medicare Advantage PPO ID Card Sample

Member ID:

Your unique identification number. Always keep it confidential.

Group Number:

This number identifies your specific benefits plan.

Provider Name/Network:

Indicates the name of your PCP or network of providers.

How to Read Your Card

Front Side

The front of your card contains your Member ID, Group Number, and Provider Network.

Back Side

Flip the card to find emergency contact numbers and prescription benefits (if available).

Using Your Card for Medical Services

At the Doctor’s Office

  1. Check In: Present your card at the reception.
  2. Verification: The office will verify your coverage.
  3. Payment: Co-payments are made at this time, if applicable.

At the Hospital

  1. Admission: Show your card at the admissions desk.
  2. Verification: Your coverage will be verified.
  3. Billing: Any required payments or co-payments will be processed.

  1. Referral: Some plans require a referral from your primary care physician.
  2. Appointment: Schedule your visit and present your card upon arrival.

Using Your Card for Prescription Services

At the Pharmacy

Become a WellPATH Champion

I agree to represent my school/department as the Wellness Champion for this school year. I understand the outlined roles and responsibilities of the Wellness Champion and will assist in the implementation and coordination of wellness initiatives. I agree to not only support the WellPATH Employee Wellness program by my participation, but to collaborate with my local management team to support the initiatives as well. I understand that the success of the program hinges on all employees being empowered to make informed decisions that positively impact their health. The WellPATH program can count on me to be a messenger and motivator and to help gather information from my co-workers, principals and managers to help foster a culture of wellness within Fresno Unified School District that supports good health.

I also understand that I may be asked to assist in scheduling rooms and programs (including wellness screenings, seasonal influenza program) at my location, displaying WellPATH brochures, posters, etc. in appropriate areas, and encouraging my co-workers to be involved in the WellPATH Employee Wellness program.
