The Joint Health Management Board (JHMB) is committed to providing comprehensive healthcare benefits to our members. In line with our mission to promote informed and proactive decisions, we are pleased to announce new value-added features for Delta Dental enrollees.

The Delta Dental plan doesn’t just cover dental services—it’s about your overall health. If you are a Delta Dental enrollee, you now have access to preferred pricing on LASIK eye surgery through QualSight and hearing aids through Amplifon.

What Are the New Features?

Delta Dental, in collaboration with QualSight LASIK and Amplifon Hearing Health Care, is offering the following preferred pricing on LASIK eye surgery and hearing aids:

  • QualSight LASIK: Receive a 35% discount off the national average price of LASIK eye surgery.
  • Amplifon Hearing Aids: Enjoy an average of 66% savings off retail hearing aid pricing.

Who Is Eligible?

All Delta Dental enrollees are eligible for these value-added features. Whether you are an active employee, a retiree, or a family member covered under a Delta Dental plan, these benefits are designed to enhance your overall well-being.

How Do You Participate?

Follow these directions to access these new benefits.

Amplifon Hearing Aids

  1. Call Amplifon at 888-779-1429 for hearing aids.
  2. Receive guidance from a patient care advocate who will explain the program, help you find a hearing care provider, and help you make an appointment.
  3. Receive program activation details from Amplifon, which will send you and your provider the necessary communication.
  4. Save on hearing aids and receive either complimentary batteries for two years or a complimentary standard charging station.

QualSight LASIK Eye Surgery

  1. Call QualSight at 855-248-2020 for LASIK services.
  2. Receive guidance from a care manager who will explain the program, answer any questions, help you pick a provider, and set up a free consultation to determine eligibility for LASIK eye surgery.
  3. Receive written confirmation from QualSight, including pricing information and directions to your provider’s office.
  4. Pay the contracted price for LASIK services.

Become a WellPATH Champion

I agree to represent my school/department as the Wellness Champion for this school year. I understand the outlined roles and responsibilities of the Wellness Champion and will assist in the implementation and coordination of wellness initiatives. I agree to not only support the WellPATH Employee Wellness program by my participation, but to collaborate with my local management team to support the initiatives as well. I understand that the success of the program hinges on all employees being empowered to make informed decisions that positively impact their health. The WellPATH program can count on me to be a messenger and motivator and to help gather information from my co-workers, principals and managers to help foster a culture of wellness within Fresno Unified School District that supports good health.

I also understand that I may be asked to assist in scheduling rooms and programs (including wellness screenings, seasonal influenza program) at my location, displaying WellPATH brochures, posters, etc. in appropriate areas, and encouraging my co-workers to be involved in the WellPATH Employee Wellness program.
