The District’s Joint Health Management Board (JHMB) continuously strives to build and maintain a comprehensive benefits program for our employees and their families. Each year, the JHMB reviews various aspects of the benefit plan to ensure our plans address the needs of our members.

Most recently, the JHMB approved a change to the district-paid Life and Accidental Death & Dismemberment (AD&D) Insurance benefits for all active plan participants (insured by Standard Insurance).  Previously, benefit amounts gradually decreased with age. However effective September 1, 2019, the JHMB now provides both Life and AD&D benefits equal to $30,000, regardless of age.

New Benefit Amounts Effective September 1, 2019

Please note this plan change does not affect the voluntary employee-paid additional life insurance coverage. For more details about the insurance benefit options available to you through the District, review the Benefits Information Guide (pages 19-20) available online at www.JHMBHealthConnect.com/your-benefits. You may also download a copy of the official Standard Life Insurance Certificate via the District’s internal SharePoint site at https://fusd.sharepoint.com/sites/dept_benefits/Pages/default.aspx.

REMINDER: Are Your Beneficiaries Up To Date?

In the event of an unfortunate death of an active employee, the Life and AD&D Insurance benefits provide the surviving family member(s) or other beneficiaries with financial protection and security. It is important to make sure your beneficiary designation is up to date and on file, so as to ensure the correct individuals or entities receive the payments they shall be due from your policy. If you have remarried, married, divorced, etc., please remember to update your beneficiary designation. You can change your designations at any time and you may designate a sole beneficiary or multiple beneficiaries to receive payments based on specific percentage allocations.

How to Update Your Beneficiaries: To select or update your beneficiaries, download the Beneficiary Designation Form, under the “Life Insurance” tab on the JHMB’s Benefits Forms page (http://www.JHMBHealthConnect.com/your-benefits/benefit-forms). Type your details into the PDF form, and then print and mail, fax, or deliver the completed form in-person to the Benefits Department at the information below:

Once the information is processed, it will be updated and available to view within your Employee Self-Service account online.

I understand the outlined roles and responsibilities of the Wellness Champion and will assist in the implementation and coordination of wellness initiatives. I agree to not only support the WellPATH Employee Wellness program by my participation, but to collaborate with my local management team to support the initiatives as well. I understand that the success of the program hinges on all employees being empowered to make informed decisions that positively impact their health. The WellPATH program can count on me to be a messenger and motivator and to help gather information from my co-workers, principals and managers to help foster a culture of wellness within Fresno Unified School District that supports good health.
