DATE: September 24, 2018

We Need Your Input: We’re Exploring New Ways to Provide Healthcare for You and Your Family

***Active Employees and Early Retirees Only***

The Joint Health Management Board is currently exploring the cost and operational feasibility of an employer-sponsored health center for the District’s employees and their families. The health center could provide primary care, treat acute illness and minor injuries, prescribe medications, help with chronic conditions, provide preventive care exams and focus on supporting the overall health of employees and their families. But, first, we need your input!

Complete the Survey from Sept 24 – Oct 5
Chance to Win One of Three (3) $100 Gift Cards

We are asking for your input on how, or if, you might use the health center and what services you would like to see available. Your comments in the online survey will be held confidential and will help guide the consideration for and possible development of such a health center to best meet your needs.

Details About the Employee Health Center

A Health Center: Efficient, Convenient Care

The health center would not replace your current primary care physician – rather, it could supplement that relationship. Providers in the health center would see one patient at a time, allowing for shorter wait times, as well as longer one-on-one time with providers. The health center would maintain provider-patient confidentiality while offering convenience for employees and their families, as well as the ability to customize services to meet your needs.

How it’s Different

The health center would be different from retail clinics (such as CVS Minute Clinics or Walgreen’s Take Care Clinics) because it would provide full primary care services, emphasize overall wellbeing, and may be available at a lower cost to you.

Protecting Your Privacy

The health center would not be able to share your personal health information without your approval and it would NOT be managed by the District. It would be managed by a reputable health care provider.

Become a WellPATH Champion

If your site is not listed, please include your location below.

I agree to represent my school/department as the Wellness Champion for this school year. I understand the outlined roles and responsibilities of the Wellness Champion and will assist in the implementation and coordination of wellness initiatives. I agree to not only support the WellPATH Employee Wellness program by my participation, but to collaborate with my local management team to support the initiatives as well. I understand that the success of the program hinges on all employees being empowered to make informed decisions that positively impact their health. The WellPATH program can count on me to be a messenger and motivator and to help gather information from my co-workers, principals and managers to help foster a culture of wellness within Fresno Unified School District that supports good health.

I also understand that I may be asked to assist in scheduling rooms and programs (including wellness screenings, seasonal influenza program) at my location, displaying WellPATH brochures, posters, etc. in appropriate areas, and encouraging my co-workers to be involved in the WellPATH Employee Wellness program.
